Chapter 10: Personal Assistance and In-Home Services

Section 1: Personal Care and In-Home Services

Centers for Medicare and Medicaid Services - Medicare Home Health

Baltimore MD   21244-1850

Home health care includes skilled nursing care , as well as other skilled care services, like physical and occupational therapy, speech-language pathology (therapy) services. Services may also include medical social services, and assistance from a home health aide (when needed by people also receiving skilled care).

These services are provided by a variety of health care professionals in your home. The home health staff provides and helps coordinate the care and/or therapy your doctor orders. In support of your doctor's orders, home health staff develop a care plan, which is a written plan for your care. It tells what services you will get to reach and keep your best physical, mental, and social well being. You have a right to participate in planning your care and treatment. The home health staff keeps your doctor up-to-date on how you are doing and updates your care plan as needed. Home health agencies cannot make changes to the care your doctor orders for you without your doctor's knowledge and permission.

Medicare, Medicaid, and most private insurance plans cover some medically necessary home health care. If you meet certain conditions, Medicare covers part-time skilled nursing care, physical therapy, occupational therapy, speech-language therapy, home health aide services, medical social services, durable medical equipment (such as wheelchairs, hospital beds, oxygen, and walkers), medical supplies, and other services.

The Medicare Home Health benefit provides skilled nursing and some other health services in the home. To be eligible a person must be on Medicare. To receive home health visits:

  • The doctor must approve and write a plan for your care at home;
  • You must need intermittent skilled nursing, physical therapy, occupational therapy or speech-language pathology services;
  • You must be homebound, that is, unable to leave home at all without great difficulty (usually medical trips, church and beauty shop are the only trips permissible); and
  • The agency providing your home care must be approved by Medicare.

Eligible children, ages 3 through 17, may receive up to $12,360 of services per year through the IHSW-Children. Eligible adults, 18 years or older, may receive up to $18,540 of services per year through the IHSW-Adult. These waivers are not cash payment programs and all services are provided through agencies contracting with the Oklahoma Health Care Authority. Follow this link to DHS' Community Services webpage that details Community Waivers and IHSW, www.okdhs.org/programsandservices/dd/commsvcs/.

Guardianship services are available for people receiving waiver services and for people who are residents of the Resource Centers (Northern Oklahoma Resource Center at Enid, the Southern Oklahoma Resource Center at Pauls Valley, or the Robert M. Greer Center). Services include assessment of the need for a guardian, assistance in locating a family member or volunteer to serve as guardian and vouchers for attorney's fees for the establishment of a guardian. Family Support Assistance payments - This is a cash payment program for families who are caring for children under age 18 at home. In this program, families can receive payments of $250-$400 per month depending on the number of children with disabilities in the home. Families who meet the income eligibility may choose this cash payment in lieu of the Medicaid Home and Community-Based Waiver (HCBW) Services. This program is funded through state dollars. Follow this link to DHS' webpage for the Family Support Assistance Payments, www.okdhs.org/programsandservices/dd/commsvcs/famsupp/default.htm.

Group Homes offer living arrangements for 6 to 12 people who share a home and receive up to 24 hours per day of supervision, support, and training in daily living skills. Group Home residents are 18 years of age or older and are provided community living services. Group Homes are single-family homes located in the community close to other services and activities. The home is owned or leased by a private agency. The agency receives reimbursement from OKDHS Developmental Disabilities Services Division (DDSD) for supervising and supporting the residents of the home. Follow this link to DHS' group home webpage, www.okdhs.org/programsandservices/dd/commsvcs/gphome/default.htm.
